CVCH offers quality medical,dental and behavioral health care services to families and individuals regardless of their ability to pay. If you do not have insurance they offer a sliding fee schedule to those that qualify. When making your appointment ask to speak with one of their patient services coordinators if you are unable to pay for your visit. You will need to bring proof of income for the past 30 days or your most recent tax returns to apply for this program.
