Our PCMH is committed to providing same day appointments, and offering expanded hours to meet your needs. We will use the electronic medical record to support the best care, quality, and safety. This electronic system will help us identify and provide for the needs of all our clinic patients such as sending you reminders about needed preventive and chronic care, and medication management

Don't have insurance? Don't worry! Our onsite dental services are available to you. Additionally, we can help coordinate additional dental services offsite.
At Affinity Health Center, we believe financial challenges shouldn't stop you from getting the health care you need. Our staff can offer financial assistance in a number of areas, and help make sure that the quality care you need comes at a price you can handle. We offer discounted fees for our services and function on a sliding fee scale.

HIV/STD Testing
We offer free HIV and STD testing, as well as pregnancy testing and HIV/STD prevention education programs for the entire community. Our HIV and STD testing is available by walk-in (no appointment necessary), Monday-Thursday from 9am-4pm. We provide rapid HIV testing where you receive your results while meeting with a prevention counselor. For more information about our testing program call 803-909-9726.
Affinity Health Center operates under a sliding fee scale payment model. What this means to you is that Affinity Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
