Patients must make an appointment prior to being seen at the clinic. Patients are seen Monday through Thursday between 9:00 am and noon and Thursday evenings between 5:30 pm and 7:30 pm. Patients must have an appointment.
If you are a new patient, please bring a photo ID, proof of address and proof of household income to your first appointment. We must have this information before you can be seen.
