We welcome CareOregon, Family Care, OHP (Open Card), Healthy Kids, Lifewise, Moda Health and PacificSource members, and offer a sliding fee scale based on income and family size for patients with no health insurance.

Service description:
This clinic provides urgent medical care for all ages. This clinic can treat respiratory, bladder and ear infections, skin rashes and simple foot problems. Pediatric Clinic on 3rd Saturday of each month, through March 2011, treating children up to age 18 years. Well-child checks, physicals and pediatric immunizations.

Clinic usually can give the patient the medication they need at the time of service or provide low-cost prescriptions. Patients with diabetes, hypertension, or thyroid conditions can have their medications refilled once while they are waiting to get into a primary care clinic or are between insurance coverage/jobs. This clinic does not provide mental health, drug and alcohol treatment, gynecology or family planning services, or STD screenings.
Intake procedure:Call for appointment Monday-Wednesday 9am-4pm for same week appointments.
Fees:Suggested donation of $15, but no one is refused service
Eligibility:Low income, uninsured adults and children.
Hours:Clinic: Monday/Wednesday 5:30-9:30pm; Phone: Monday-Wednesday 9am-4pm;
Occasional Monday morning appointments; Call Wednesday-Friday to schedule.

Wallace Medical Concern operates under a sliding fee scale payment model. What this means to you is that Wallace Medical Concern provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.

Obamacare purchased from the Health Care Marketplaces covers Mental Health Care and Substance Use Disorder Treatment. Under the Health Care Law there are 10 categories of benefits. Mental Health Care and Substance Abuse Treatment are two of the 10.

If you have Affordable Care Act Coverage you will have mental health care coverage.
