This Clinic is a federally qualified health center and based on a sliding scale.Sliding scale fees are based on a patient's ability to pay. The fees are reduced for those who have lower incomes, or alternatively, less money to spare after their personal expenses, regardless of income.

What this means to you is that Heartland Community Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
