Heartland health centers have a sliding scale for those who do not have health insurance. This fee will include visits to the provider, radiographs and laboratories, if requested. The sliding fee scale is based on family size and income. Proof of income is required for the first visit. If you have no income, a notarized letter from the person supporting you is required. If you are prescribed medication, the staff will help you get medicines at an affordable price.
