Circle offers a sliding fee scale for services based on income, so everyone is assured of being treated.

WHAT TO BRING:
Payment, proof of income, or proof of insurance is expected at the time of service. Bring this information each time you or your family members visit the health centers.
Patient Support staff can assist you in applying for health insurance through special programs such as County Care if you qualify. Please call and ask to speak with the Financial Counselor.
If you do not qualify for any of the programs listed, you may still qualify for a complete medical visit for a affordable price, labs will be extra. Bring a check stub or other proof of income to apply for reduced-fee services.
Mental health services are also provided on a sliding-fee scale plan.

Sliding Scale

Parkside Health Center operates under a sliding fee scale payment model. What this means to you is that Parkside Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.

Affordable Care Act (Obamacare) Insurance Coverage For Mental Health

Obamacare purchased from the Health Care Marketplaces covers Mental Health Care and Substance Use Disorder Treatment. Under the Health Care Law there are 10 categories of benefits. Mental Health Care and Substance Abuse Treatment are two of the 10.

If you have Affordable Care Act Coverage you will have mental health care coverage.
