Heartland Health Centers has a sliding-fee scale for those that do not have health insurance.
This fee will include the visit with the provider, x-rays and labs, if ordered. The sliding-fee scale is based on household size and income. Proof of income will be required at your first visit. If you have no income, a notarized letter from the person supporting you will be required. If you are prescribed medication, our staff will work with you to obtain medicine at an affordable cost.

What this means to you is that Heartland Health Center Albany Park - Community Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
