Medical Services:

Primary Care Providers (PCPs) at Heritage Health have a broad set of skills and are able to diagnose and treat most conditions and ailments.

Chronic Disease Management: Includes conditions like hypertension, diabetes, pain and other chronic diseases.

Diagnostic Services: X-ray services, ultrasounds, stress tests, etc.

Express Care: For patients suffering from acute, but non-life-threatening conditions like sprains, cuts or ear infections, Express Care is the same day, no appointment, walk-in option.

All patients are eligible for our sliding fee scale. A sliding fee scale means that fees charged to you for services provided may be discounted based upon household income and family size. These discounts are available to patients based on the guidelines provided annually by the federal government.
