The Baker Family Medical Center provides medical services for the entire family . The clinic accepts most insurances and also offers a discount sliding fee program to those that qualify . To apply for the sliding fee program you will need to fill out an application and provide proof of income and family size information .The applications is available on the website on the appointments page .

When going to your first appointment you will need to bring Photo ID and/or Drivers License , Insurance Card if applicable , List of Current Medications & dosage or bring medications with you ,New Patient Packet, Sliding Fee Scale Application (If applying for Discount Sliding Fee),Proof of Income - (If applying for Discount Sliding Fee).

Clinical Services: General Primary Medical Care, Diagnostic Laboratory, Screenings •Cancer, •Communicable Diseases, •Cholesterol, •Blood lead test for elevated blood lead level, •Pediatric vision and hearing, Non-Emergency Medical Services, Voluntary Family Planning, Immunizations, Well Child Services, Gynecological Care, School / Sports / Work Phycials, Referral to Mental Health, Referral to Substance Abuse, Referral to Specialty Services

 

Non Clinical Services: Counseling/Assessment, Referral, Follow - up / Discharge Planning, Eligibility Assistance, Health Education, Outreach

Baker Family Medical Center operates under a sliding fee scale payment model. What this means to you is that Baker Family Medical Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.

Obamacare purchased from the Health Care Marketplaces covers Mental Health Care and Substance Use Disorder Treatment. Under the Health Care Law there are 10 categories of benefits. Mental Health Care and Substance Abuse Treatment are two of the 10.
