The Nassau County Department of Health offers primary care services to residents of Nassau County.
This means that they would be the medical home for Nassau County residents who meet the financial, residency and diagnostic requirements of the primary care program.


To qualify for a primary care program, you need to:

Financially qualified.

The poverty level must be at 150% and you will have to show proof of income.

High blood pressure

High Cholesterol

Asthma requiring daily treatment

Chronic obstructive pulmonary disease (COPD)


Hypothyroidism

Their primary care program does not provide services for those who need only chronic pain management or behavioral / mental health services, and they advise those in need of these types of services to seek assistance through specialist clinics.
