This program assesses people with mental illnesses for service needs and links them to the needed community resources such as social security, Medicaid/Medicare and food assistance. ACM also helps people plan short and long-term recovery goals while monitoring progress on reaching their recovery goals. ACM also serves as an advocate in the individuals recovery.
