Health Care for the Homeless
 
Health Care for the Homeless (HCH) provides health care services for homeless individuals and families throughout Maricopa County. We deliver quality health care to homeless people through street outreach, integrated primary care, mental health services, substance abuse services, case management, and nutrition services.

We offer the following services:
 
    Medical treatment
    Nutrition counseling
    Substance abuse treatment
    Mental health services
    Case management
    Referrals for additional social services
    Emergency, transitional, and permanent housing
    Employment assistance
 
Location
The Health Care for the Homeless clinic is located on 12th Avenue, south of Madison Street in Phoenix.
